Job SummaryThe Prison Aftercare Specialist supports formerly incarcerated individuals in their transition back into the community and justice-involved women in the community. This role involves developing and implementing aftercare plans, connecting clients with resources, and providing ongoing support to promote successful reintegration and reduce recidivism.Key ResponsibilitiesAssessment and PlanningConduct comprehensive assessments of individuals' needs, strengths, and challenges immediately post-release.Develop personalized aftercare plans in collaboration with the individual and relevant stakeholders.Set realistic goals and objectives to address identified needs and promote successful reintegration.Case ManagementProvide one-on-one case management and support to individuals throughout their reentry process.Maintain accurate and timely documentation of all interactions, assessments, and services provided.Monitor progress toward goals and adjust aftercare plans as needed.Resource CoordinationConnect individuals with community resources, including housing, employment, healthcare, education, and counseling.Maintain up-to-date knowledge of available resources and services in the community.Coordinate with other service providers to ensure a holistic approach to aftercare support.Support and AdvocacyProvide emotional support and guidance to individuals navigating post-release challenges.Advocate for individuals to ensure they receive appropriate services and support.Assist individuals in overcoming barriers such as lack of identification, transportation, or access to basic needs.Education and Skill BuildingFacilitate workshops and training on topics such as job readiness, financial literacy, and life skills.Provide information and resources on rights and responsibilities post-release.Empower individuals with tools and strategies to make positive choices and avoid recidivism.Collaboration and OutreachBuild and maintain strong relationships with community organizations, employers, service providers, and stakeholders.Participate in community outreach activities to promote awareness of aftercare services and support.Attend meetings, trainings, and professional development opportunities to stay informed about best practices in aftercare.Follow-Up and EvaluationConduct follow-up visits and check-ins to ensure ongoing support and stability.Evaluate the effectiveness of aftercare plans and adjust strategies as necessary.Collect and analyze data to measure outcomes and improve aftercare services.QualificationsEducationBachelors degree in social work, criminal justice, psychology, human services, or a related field (required).Masters degree in social work or a related field (preferred).ExperienceMinimum of 2-3 years of experience in aftercare services, social work, corrections, or a related field.Experience working with justice-involved individuals and knowledge of the criminal justice system.SkillsStrong interpersonal and communication skills.Ability to work independently and as part of a team.Excellent organizational and time-management skills.Proficiency in case management and documentation.AttributesEmpathy, patience, and cultural sensitivity.Commitment to empowering individuals and promoting their successful reintegration.Flexibility and adaptability in a dynamic work environment.Work EnvironmentTypically works in an office setting, with frequent travel to meet with clients in community locations or their homes.It may involve evening or weekend hours to accommodate client schedules.Some work will be done inside the female prison (Tutwiler, Montgomery Womens Facility, Birmingham Work Release)Mandatory to work on the 2nd Saturday of every monthJob Type: Full-timePay: $35,000.00 per yearBenefits:Dental insuranceFlexible scheduleHealth insurancePaid time offSchedule:Monday to FridayWeekends as needed
