St. Johns Ministries (SJM) is seeking a Case Manager to serve this faith-basedorganization and uphold its mission to honor the dignity, restore hope and create lastingchange for those experiencing homelessness or housing instability in the greater GreenBay community. St. Johns Ministries services are robust and include two seasonal emergency shelters oflast resort, where we provide adults experiencing homelessness with a safe place to sleepduring the harsh winter months. We also offer two daytime resource centers, the MicahCenter and Wellspring, where we equip adults experiencing homelessness, as well ascommunity members at-risk, with the tools to achieve self-sufficiency. Life-saving MissionSupporting a person in their journey to self-sufficiency is the goal of St. Johns Ministriesyear-round daytime resource centers. Regardless of where they reside, individuals haveaccess to on-site medical care, mental health treatment, support groups and workshopsdesigned to eliminate barriers and ensure they have the tools necessary to move towardstheir self-sufficiency goals. St. Johns offers on-site computer labs, daily meals, and one-on-one case management.Further, Micah Centers offers the Joseph Project, an employment training program inpartnership with Senator Ron Johnsons office, to prepare individuals for full-time jobsoffering a living wage. The women-specific daytime resource enter, Wellspring, offerssafety and belonging to women who are homeless, living with mental illnesses, disabled,facing financial issues or in broken relationships. Wellspring strives to elevate the mind, body,and spirit through peer support, personal growth opportunities, resources, and referrals.St. Johns fosters an atmosphere of dignity and respect as they empower women and providethe means to help them grow. The Micah Center and Wellspring are important resource centers for those who may befacing eviction or job loss. Since even one episode of homelessness can lead to decreasedlife expectancy, St. Johns Ministries work of prevention is truly a life-saving mission. St. Johns ServicesShelter. A place to sleep and a warm meal is available for those that qualify at one of ourtwo seasonal emergency shelters of last resort. St. Johns only serves adults that arent eligibleanywhere else and who would otherwise be sleeping on the streets. Our sheltersoperate from November 1 - April 30. Resources. Supporting a person in their journey to self-sufficiency is the goal of St. Johnsyear-round daytime resource centers. Individuals have access to support groups andworkshops, designed to eliminate barriers and ensure they have the tools necessary tomove towards their self-sufficiency goals. Case Management. Each guest of St. Johns Ministries is assigned a case manager to assistwith barriers faced and to work toward goals that will eventually lead to self-sufficiency.Case Managers provide guidance, resources and referrals to programs that will lead toemployment and potentially housing. Role and Responsibilities:The primary focus of this full-time position is to coordinate supportive, appropriate community services forguests in a manner that helps them move forward in their lives. Plan and implement workshops that assist guest in addressing life barriers and taking steps towards self sufficiencyCarry a client caseload and meet with assigned guest on a regular basis to address goals and barriersAccurately record data and guest notes, perform and routinely assess guest intake, perform all reviews, coordinate referral appointments, and assist on-site agencies in services to guestRoutinely review and update guest charts for services, behaviors, needs, issues, history, trends and follow through on case management. Notify Associate Executive Director of concerns or issues requiring additional attentionTrack guest work schedules, appointment, medication schedules, and compliance with shelter rules. Record incident report, neighborhood concerns, and guest concerns.Effectively handle guest and staff corrective behavior in discreet and respectful mannerFacilitate staff and guest meetings/trainings as needDevelop and maintain case management schedule for individual guest meetingsEnter all pertinent guest information into guest record, daily reports in Case Worthy Typical Working Hours are Monday-Friday between the hours of 8am-5pm, all case managers are required to work 1-2 evenings per week in shelter between the months of November-April. Qualifications and Education RequirementsBachelor's Degree in Social Work or Human Services with appropriate job experienceAccurate and complete recording of dataExcellent Computer Skills: Internet, Micro Soft ProductsExperience working with: mental health, substance abuse, variety of behavior, low income and/or homeless population, and health careAbilities:Maintain confidentialityEffectively work cooperatively with othersDemonstrate positive attitudeComplete job duties accurately and effectivelyProvide client advocacy and support effectivelyEstablish and maintain professional boundaries with guest, volunteer and co-workersFamiliar with community resources, refer guest appropriately and tracks referralsCommunicate corrective behavior needs, actions, and consequences to staff and guest in a clear, positive, and discreet manner with a focus on desirable behaviorInitiate and compete case management duties effectively with minimal supervision Physical/Visual/Mental Demands:Position require ability to respond effectively to staff, guest, collaborative agencies and volunteer with questions and concerns in areas of responsibility. Must have an excellent command of computer software programs.
