The Home Care Aide/Certified Nursing Assistant is a member of the Nurse Next Door team and works under the supervision of the Care Designer or a nurse and performs personal care services contained in a care plan written by a licensed health professional, employed or contracted by the agency. The Home Care Aide/Certified Nursing Assistant is responsible for observing clients, reporting these observations and documenting observations and care performed.The Home Care Aide/Certified Nursing Assistant will be assigned in a manner that promotes quality, continuity and safety of a clients care.Responsibilities of the Certified Nursing Assistant include, are to, but are not limited to the following:Provide client care as directed by the Care Designer or Nurse.Personal care: a. Bathing b. Grooming, including oral careAssist the client with dressing and undressingAssist the client with toileting activities, including the use of the bedpan and urinalProvide catheter care including changing the urinary catheter bagEmpty ostomy bags, or changing bags that do not adhere to the skinAssist in feeding the client, if necessaryFeed clients who are unable to feed themselvesTake and record vital signs (temperature, pulse, respirations and blood pressure)Measure and record height and weight, if necessary.Support good skin care to prevent skin breakdown (i.e. turn and reposition client, activelyencourage movement)Assist client with active and passive range of motions as defined by the plan of careProvide client with help walking and transferring; assist with the use of devices for aid to daily living such as a wheelchair or walker;Observe clients physical, mental and emotional conditionsi. Document observed changesMeasure and document intake and output, if necessaryAssist client in the self-administration of medicationVerbally remind clients to take their medicationsPerform light housekeeping; maintains a clean, safe and healthy environment, which mayinclude light cleaning and straightening of the bathroom, straightening the sleeping and living areas, washing the client's or client's dishes or laundry, and such tasks to maintain cleanliness and safety for the client;Perform meal preparation and clean upChange bed linens and make up the clients bedMaintain an orderly client environment.Follow principles of infection control and Universal Precautions.Identifies and participates in performance improvement activities.Provide transport to doctor appointments, outside activities and shopping.Participates in client teaching according to the clients plan of care.Assist in providing a safe environment and promote quality client care through adherence toestablished policies, procedures and standards.Ensure clients rights are adhered to. These include but are not limited to:the right to privacy and dignity while care is being providedthe right to accept or refuse carethe right to confidentiality of their informationRespond rapidly and appropriately in an emergency situationUse equipment properly and in a safe manner.Perform other tasks as determined and instructed by the licensed professional and as permissible under Idaho law.
