Position Summary
As a Health Coach, you'll provide one-on-one HIPAA-compliant coaching via 30-minute telephone, video, and instant message to members seeking improved digestion and relief from chronic conditions. You'll use coaching skills like Motivational Interviewing, strengths-based care, and the transtheoretical model to provide member-centered care based on the member's primary concern. Most of the time, you'll help members with stress management, mindfulness, healthy sleep, medication adherence, self-advocacy, physical activity, self-monitoring, and assistance with the app.
We guarantee 15 hours/week with the expectation to flex up to 28 hours/week 4-6 months of the year. On average, our health coaches are working 20-28 hours a week - peak times are often the beginning of the year or when we launch large clients.
Note: Starting pay will be based on a number of factors and commensurate with qualifications & experience and ranges between $24.00-$26.00 per hour plus differential pay for part-time ($2/hr).
Responsibilities Include:


Acting as the main point of contact for members, coordinating ‘whole person’ care, and helping the members navigate through our chronic disease management program


Using coaching skills (MI, TTM, strengths-based care) to help members move through the stages of change via one-on-one HIPAA-compliant telephone, video, and instant messaging


Assessing and providing member-centered health coaching based on the member’s greatest concern. You must feel comfortable coaching on topics that include, but are not limited to: stress management, mindfulness, healthy sleep, medication adherence, physical activity, and self-monitoring


Referring the member to our Registered Dietitians for Medical Nutrition Therapy (MNT) based on MNT escalation plan


Aiding in the administrative burden of caring for a chronic disease and supporting the member with developing self-efficacy and self-advocacy


Applying our Member Engagement Model and Clinical Pathways to promote symptom relief, digestive condition treatment, and behavior change


Providing education, setting SMART goals, and acting as an accountability partner to help members feel their best


Documenting according to established guidelines and expectations


Collaborating with the interdisciplinary team for optimal care for the members


Practicing within HIPAA/state/federal guidelines, our Clinical Pathways, within role scope of practice, and referring to other disciplines according to our escalation plans and guidelines


Maintaining strict confidentiality of members’ Protected Health Information and is bound by our confidentiality agreement


You'll be a good fit here if you are:


Technologically savvy, you must be able to pick up new technology quickly and seamlessly


Outstanding with your interpersonal and communication skills, specifically your ability to engage in a dynamic clear conversation by telephone or video


Knowledgeable and have experience in integrating behavioral modification methodologies/techniques into coaching delivery


Self-directed, highly organized, and able to multitask


Required Qualifications:


A National Board Health and Wellness Coach (NBHWC) or eligible to sit for the exam within six months of hire. You must have a Bachelor's degree in a health-related field


A minimum of one year of face-to-face, telephonic, or video health coaching experience. This does not include chat-based coaching


Flexibility to work with variable individual schedules across multiple time zones, including one evening until 8 Central Time. Your schedule will be created two months in advance
