Develop and implement person-centered housing plans based on individual assessments Reassess and update housing plans annually to reflect changes in needs and preferences. Support individuals in identifying their strengths, needs, and preferences related to housing, including cultural requirements. Assist individuals in making informed decisions about housing choices and transitions. Educate individuals about their rights to privacy and the appeal process. Coordinate with other service providers to ensure comprehensive support. Develop and support housing transition plans, including benefits application, housing search, and lease negotiation. Assist with tenant screening, budgeting, and organizing the moving process Facilitate cultural practices and ensure prospective landlords and property managers respect these needs. Ensure new living arrangements are safe, accessible, and ready for move-in. Research and contact potential housing options and assist with application completion. Monitor for behaviors that may threaten housing stability and take preventive measures. Conduct and review background checks as part of tenant screening and housing applications. Identify and secure resources for housing-related expenses, such as deposits and moving costs. Preventing and early identification of behaviors that may jeopardize continued housingQualifications: Bachelor's degree in Social Work, Human Services, or a related field. Experience in housing support, case management, or a similar role. Strong understanding of housing regulations, benefits, and application processes. Excellent communication and organizational skills. Ability to work flexible hours, including evenings, weekends, and holidays. Ability to work collaboratively with individuals from diverse backgrounds and with variousneeds. Must pass a background check and meet other regulatory requirements under the 245D Waiver.Physical Demands: Ability to lift and transfer clients as needed. Perform tasks requiring physical stamina, such as standing, walking, and lifting. Work Environment: Work may be performed in clients' homes, community settings, or Iska Inc. facilities. Exposure to various household conditions, including pets, cleaning products, and potential allergens. Staff must successfully complete background checks and obtain clearance as mandated by theDepartment of Human Services (DHS).
