Personal Care Worker

Job descriptionJob Overview:We are seeking a compassionate and dedicated Personal Care Worker to join our growing team, providing care and support to our individual with mental health and individuals with disabilities in our Adult Family Homes. The ideal candidate will be responsible for assisting with daily living activities and promoting independence and well-being.Responsibilities:- Develop and implement personalized care plans to meet the unique needs of each client.- Provide assistance with activities of daily living (ADLs) such as bathing, dressing, and grooming.- Offer support with medication management and patient monitoring.- Engage in patient observation to identify any potential health issues or concerns.- Deliver high-quality care, recognizing the unique challenges and needs of individuals with severe mental health.- ensuring that clients receive the support and attention they need to thrive.- Provide group home care, creating a welcoming and supportive environment for clients.- Maintain accurate records and documentation of client care and progress.Experience:The successful candidate will have relevant experience in a healthcare setting, with a strong focus on care and support for seniors and individuals with disabilities.
